Activity / Announcement Request Form (ARF)

1. Fill out at least 4 weeks prior to your activity.

2. Before submitting this form, it is wise to verify that the dates and times you choose do not conflict with another church activity that is on the calendar. Contact info@valleyvegas.org if you need assistance.

3. The activity or announcement must be approved by the church office administration before it will be placed on the church activity calendar or announced through any church publications.

4. Do not consider your activity or announcement approved until you receive a text or email stating that it has been approved.

5. Read the Valley Vegas Activity Rules and Guidelines at the bottom of this form.

General Rules And Guidelines

1. The Church reserves the right to make any changes and/or cancellations to any approved scheduled activities or announcements.
2. Only Ministry leaders may coordinate and plan activities involving the use of the church.
3. No smoking of any kind, including e-cigarettes, or drinking alcoholic beverages while on the premises.
4. The approved area and any surrounding locations used during the activity must be thoroughly cleaned and vacated by the approved activity stop time, including cleaning any bathrooms used during the activity.
5. Only the approved rooms and space you have reserved may be opened and used.
